About the Role

Austin Health is seeking a highly skilled and dedicated professional to join our Adolescent Inpatient Unit as an Intake and Transitions Clinician.

Key Responsibilities
  • Manage all incoming referrals to the adolescent inpatient unit, ensuring timely and effective admissions.
  • Collaborate with the interdisciplinary team and external stakeholders to ensure seamless transitions and discharge planning.
  • Support consumer flow through the inpatient unit, ensuring safe and successful discharge.
  • Ensure relevant information management systems are up to date and accurate.
  • Complete post-discharge contacts within required timeframes.
Requirements
  • Relevant professional qualification in a health-related discipline (Nursing, Social Work, Occupational Therapy, or Psychology) and current registration with the relevant professional board.
  • Demonstrates a flexible, positive, and innovative team-oriented approach to service delivery and change.
  • Strong communication and interpersonal skills, with the ability to build and maintain strong working relationships with internal and external stakeholders.
  • Strong organisational and time management skills, with a dynamic and flexible approach to caseload demands.
