Job Description:

We are seeking a highly skilled and dedicated Support Coordinator to join our team. The ideal candidate will be responsible for connecting participants with NDIS plan to supports and services in the community, supporting and assisting within Australia's NDIS participant's abilities to connect to and coordinate informal, community and funded supports in the service delivery environment.

Key Responsibilities:
  • Work with participants to find providers, services and support to achieve their goals as per their NDIS plan
  • Resolve points of crisis and develop resilience with all stakeholders
  • Involve participant and family in implementation of individual packages where appropriate
  • Active participation in Professional Development
  • Deliver services and support with a high level of quality and satisfaction
  • Attend planning and review meetings as requested
  • Accurate and complete records of work activities in accordance with legislation and operational requirements
  • Liaise and report to NDIA and associated organisations

Required Skills and Qualifications:

  • Tertiary qualifications in a relevant field, such as Social Work, Service Coordination, Case Management, Allied Health and/or Disability-specific Qualifications at associate diploma level
  • Current Australian Drivers Licence
  • Current NDIS Worker Check
  • Current Working with Children Check
  • At least 2 years' experiences within the disability sector
  • Understanding of NDIS Legislation and Code of Conduct
  • Problem solving and negotiation skills
  • Ability to manage time effectively and work under pressure
  • High level interpersonal, communication and teamwork skills
  • Well-developed oral and written communication skills
