Job Description

We are seeking a Support Specialist to join our team at a local disability and aged care service provider. As a Support Specialist, you will play a vital role in ensuring that clients receive the best possible support to achieve their goals.

The ideal candidate will have strong verbal and written communication skills, be passionate about supporting individuals and families, and be eager to learn and grow within the company.

Key Responsibilities
  • Manage an assigned caseload of clients requiring support under the NDIS framework.
  • Engage with clients to develop and implement personalized support plans that meet their needs and goals.
  • Conduct regular goal tracking and risk assessments to ensure continuity of care.
  • Provide information and advice to clients regarding their NDIS plans and associated services.
  • Collaborate with internal and external stakeholders to maintain continued support for clients.
