As a Community Care Specialist, you will provide tailored care that empowers consumers to maintain their independence while upholding their dignity and well-being. Key responsibilities include assisting with daily living tasks, meal preparation, household duties, wellness activities, and transportation to appointments.
Key Responsibilities:
- Personalized Support: Deliver comprehensive care that addresses each consumer's distinct needs, from personal assistance and respite to housekeeping and companionship.
- Consumer Advocacy: Monitor and report on consumer conditions to ensure services align with their care plans.
- Communication & Coordination: Foster open communication among Sundale team members, healthcare professionals, and external service providers.
- Record Keeping: Establish and maintain accurate, confidential consumer reporting systems.
Essential Skills & Qualifications:
- Experience in providing personal care, household duties, and working within aged care services.
- Knowledge of infection control, dementia care, and Work Health & Safety principles.
- Strong interpersonal communication skills, with the ability to work collaboratively within a flexible team environment.
- Certificate III in Home, Community Care, Aged Care or Individual Support (desirable)
- Police Check
- Pre-employment medical
- Current Queensland Driver's License
- A safe and reliable vehicle suitable for transporting Sundale consumers and their associated equipment
Availability:
Varying shifts between 6:00am and 8:00pm, working across 7 days a week, including evening and weekend shifts on rotation.
