Job Title

The Intensive Education Case Manager plays a crucial role in delivering high-quality case management and transition support to young people at risk of disengagement from education.

This position requires effective collaboration with multi-disciplinary teams, including government and non-government organisations, families, education providers, community organisations, and other key stakeholders.

The successful candidate will establish and sustain strategic partnerships, facilitate the coordination of individual student plans and transition plans, and implement best practice, culturally appropriate case management.

  • Deliver intensive case management, provide transition supports, and manage education initiatives relating to students at risk of disengagement or involved with the youth justice system.
  • Provide case management that may require educational and behaviour intervention.
  • Establish and sustain effective partnerships with stakeholders, including families, education providers, community organisations, and other key stakeholders.
  • Facilitate the coordination of individual student plans and transition plans using an integrated approach that considers a student's strengths and presenting concerns.
  • Implement best practice, culturally appropriate case management for all students to improve learning outcomes.
Essential Skills:
  • Understanding the impact of disability on individual functioning within an education context and evidence-based practices to support reasonable adjustments.
  • Experience working with diverse communities, families, and students, as well as students from culturally diverse backgrounds.
  • Experience working with young people who demonstrate a range of support needs.
