Dynamic Case Management Role

As an Advanced Practitioner, you will play a vital role in delivering dynamic case management practice to support individuals who have been imprisoned.

Our team is dedicated to helping these individuals make positive changes in their lives and reducing reoffending in the community. We are seeking a skilled practitioner to join our team and contribute to this mission.

  • The ideal candidate will have demonstrated experience in effective case management practices with complex clients, excellent verbal communication skills, and confidence in dealing with challenging behaviours.
  • They will also be able to model pro-social behaviours to a caseload of individuals, lead and mentor staff to achieve their best, and develop quality evidence-based case management practice.

Working as part of a team, you will conduct risk assessments, prepare case plans, provide interventions, and coordinate service providers and care teams. Your daily responsibilities will be varied and challenging, but also rewarding and fulfilling.
