Job Description:

We are seeking a skilled Clinical Care Coordinator to join our team. In this role, you will be responsible for coordinating clinical services to support clients in their own homes.

As a Clinical Care Coordinator, you will work closely with healthcare professionals and other stakeholders to ensure seamless delivery of care services. Your main duties will include daily coordination of care for new and existing clients, acceptance of referrals, and identifying clients eligible for government funding streams.

Our ideal candidate will have a Bachelor of Nursing degree, experience in acute nursing, community nursing, and strong problem-solving and communication skills.

  • Strong organizational and time management skills
  • Effective communication and interpersonal skills
  • Ability to work independently and as part of a team

The successful candidate will be able to work over 7 days, possess a full driver's license, and have previous experience managing service contracts and staff.
