Clinical Service Coordinator

This is a full-time position that involves coordinating clinical services to support clients in their own homes.
As the primary point of contact, you will work closely with clients and staff to ensure efficient service delivery and achieve the best possible outcomes.
Main Responsibilities:- Coordinate daily care for new and existing clients of the SA Health Community Care Program.
- Accept referrals from the Metropolitan Referral Unit (MRU) and ensure client information is available for service commencement within required timeframes.
- Identify eligible clients for Government funding streams and liaise with referrers, MRU, or healthcare professionals for care transfer.
- Bachelor of Nursing degree.
- Experience in Acute Nursing.
- Community nursing background.
- Full Driver's License.
- Previous experience managing service contracts.
- Previous experience supervising staff.
- Strong problem-solving and time management skills.
- Effective communication skills.
- Computer proficiency.
- Able to work over 7 days on a roster.
- Logistical experience (an asset).
