  • Develop and maintain effective relationships with service providers, allied health professionals, and community stakeholders to deliver high-quality outcomes.
  • Cultivate participants' capacity to manage their own supports over time, fostering independence and self-empowerment.
  • Monitor and report on plan budgets, ensuring services are delivered efficiently and effectively.
  • Provide regular progress updates to participants and relevant parties, maintaining open communication channels.

Requirements

  • Demonstrated ability to work independently and collaboratively as part of a team.
  • Excellent interpersonal and communication skills, with a focus on building strong relationships.
  • Able to navigate complex systems and processes with ease, ensuring timely and effective delivery of services.
