About the Role

The Homelessness Support Coordinator will provide clients with three years of intensive case management support, working from a strengths-based, trauma-informed approach to support clients to address unmet needs and participate in the community.

This role will include conducting capacity and independent living assessments and making recommendations on any aids/services that will assist clients to remain at home.

Key responsibilities:

  • Collaborative assessment and intervention planning
  • Linkage to specialist services including healthcare, education, employment, and housing
  • Maintenance of accurate records and reports as required
  • Other related duties as required by the Manager

Key Selection Criteria

  • Demonstrated understanding of homelessness issues and service delivery models
  • Proven experience in case coordination or a related field
  • Strong communication and interpersonal skills
  • Able to work effectively in a team environment
  • Ability to maintain confidentiality and handle sensitive information
