Medical Care Specialist

This position involves delivering medical care to patients in their homes using virtual care technology and face-to-face clinic appointments.

The successful applicant will work collaboratively as part of a team to provide timely communication between healthcare services, including NSW Ambulance, Emergency Department, Hospital inpatient services, Residential Aged Care Facilities, General Practitioners, and other key partners in the delivery of community-based health care.

Responsibilities:
  1. Participate in a 7-day roster, including evening and weekend shifts as required.
  2. Provide daily support to the care team in reviewing, triaging, and providing care to people referred to the service who are at risk of hospitalisation.
  3. Support the care team in reviewing, triaging, and providing care to people admitted into Hospital in the Home (HITH) under the care of an Admitting Medical Officer (AMO).
  4. Foster linkages with other services providers servicing the population, including primary care/general practice and the Primary Health Network.
Requirements:
  • Candidates must be a Fellow of the Australasian College of Physicians or Fellow of the Royal Australian College of General Practitioners and/or other specialist recognition.
  • Demonstrated ability to work constructively and effectively in a multidisciplinary team environment.
  • Proven leadership and supervisory skills, particularly in mentoring medical and surgical trainees, residents, and medical students.
  • Previous experience in teaching patient management would be highly regarded.
