Compassionate Care Professional

We are seeking a highly skilled and compassionate Respiratory Social Worker to join our multidisciplinary team.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to provide empathetic and personalized support to patients and their families.
They will also be able to work collaboratively with healthcare professionals to deliver high-quality patient care and develop effective care plans that address the unique needs of each individual.
A strong understanding of psychosocial issues associated with acute and chronic illness is essential for this role, as well as the ability to remain calm under pressure and think critically in complex situations.

Key Responsibilities:
• Provide counseling and case management to patients and families with illness adjustment, end-of-life care, and complex discharge issues.

• Work collaboratively with multidisciplinary teams to deliver high-quality patient care.
• Develop and implement individualized care plans that address the unique needs of each patient.
