Job Opportunity: Compassionate Care Professional
We are seeking a skilled and empathetic individual to join our team as a Compassionate Care Professional.
This role involves working collaboratively with multidisciplinary teams to strengthen connections between community services and palliative care teams, closely with hospital staff, government, and non-government organizations, and service providers.
- Providing consultation, advocacy, clinical assessments, and counseling to patients, families, and carers
- Clinical knowledge and skills to crisis work, patient flow, complex care, end-of-life care, and discharge planning for vulnerable patients
