Reconnect Program Coordinator Role

The Reconnect program provides case management services to participants exiting custody with complex needs. This position requires working closely with key stakeholders, including corrections agencies and external organizations.

Key Responsibilities:

  • Support the day-to-day operations of the Reconnect program in collaboration with the Senior Practitioner.
  • Manage a caseload of participants with multiple and complex needs, providing goal-focused and participant-led support.
  • Work independently in the community as part of an outreach capacity, promoting flexibility and adaptability.
  • Develop individually tailored case plans that are focused on achieving participant goals and improving outcomes.
