About this role

We are seeking a dedicated healthcare professional to join our team in a 0.4 FTE fixed term position until June 2026.

This new role will support the delivery of chronic disease management, helping patients navigate long-term health concerns through high-quality care planning, health assessments, and follow-up support.

  • You will work closely with our healthcare providers, allied health staff, and the broader care team to coordinate and deliver compassionate, person-centred care, supporting prevention, education, and patient empowerment.
Key responsibilities
  1. To provide high-quality nursing care to patients with chronic diseases, including health assessments, care planning, and follow-up support.
  2. To work collaboratively with the care team to develop and implement individualized care plans that meet the needs of patients.
  3. To educate patients and their families about chronic disease management, self-care, and healthy lifestyle choices.
  4. To conduct health assessments and provide recommendations for health promotion and disease prevention.
  5. To participate in quality improvement initiatives and contribute to the development of policies and procedures related to chronic disease management.
About you
  • You're a Registered Nurse who is:
  • AHPRA registered
  • Experienced in community health or general practice (3+ years)
  • Confident with CDM, care plans, and health assessments
  • Skilled in immunisations, infection control and wound care
  • Organised, collaborative, and passionate about working in diverse communities
