Key responsibilities include:

  • Supporting participants through the disability service pathway
  • Helping participants build confidence, independence, and informal support networks
  • Ensuring timely, high-quality service delivery in line with performance targets
  • Building partnerships with local organisations and promoting community inclusion through grassroots engagement
  • Using data systems to document participant interactions, monitor progress, and identify service gaps
  • Ensuring alignment with relevant guidelines and organisational standards
  • Participating in team meetings, regional planning, and initiatives that promote community inclusion and continuous improvement

Required skills and qualifications include:

  • Experience supporting people with disabilities in community services or similar settings
  • Strong understanding of disability service pathways and planning processes
  • Proven ability to manage caseloads and meet service delivery targets
  • Excellent communication, interpersonal, and problem-solving skills
  • Strong community engagement and stakeholder relationship skills
  • Experience in data entry, reporting, and case management systems
  • Ability to work independently in community-based settings
  • A commitment to person-centred practice, community inclusion, and continuous improvement

Qualifications and checks required include:

  • Qualification in community services, disability, or a related field (desirable)
  • Current driver's license and reliable transport to travel locally
  • NDIS Workers Screening Check & Working with Children's Check (or willingness to obtain)
  • Undergo a Nationally Coordinated Criminal History Check
  • Valid Australian Working Rights
  • Lived experience of disability is highly valued
