Your Key Responsibilities:
  • You'll craft personalised care plans at clients' homes, focusing on enablement and wellness.
  • You'll lead engaging group sessions promoting healthy living and self-management.
  • You'll collaborate with healthcare professionals to deliver holistic care.
  • You'll utilise digital tools for virtual coaching and support.
  • You'll build strong local networks and promote the STRC program within your community.
Your Ideal Background:
  • A degree in Nursing or Allied Health with current registration.
  • At least 3 years' experience in clinical care, health coaching, or care coordination – particularly in aged or community care.
  • A passion for health promotion, preventative care, and client empowerment.
  • Confident navigation of digital platforms for accurate documentation and communication.
  • A valid driver's licence and enthusiasm for community work.
