Care Coordinator

We're seeking Behaviour Support Practitioners to join our team in North Queensland. As a key member of our interdisciplinary care service, you'll play a vital role in delivering high-quality behaviour support services to our diverse client base.
Your Role- Conduct comprehensive assessments to identify clients' functional behaviours and support needs
- Develop tailored behaviour support plans that promote positive outcomes and enhance quality of life
- Provide specialist behaviour support interventions and strategies to address complex behaviours and promote positive outcomes
- Deliver training and support to ensure the effective implementation of behaviour support plans
- Monitor and evaluate plan effectiveness, making adjustments as needed
- Tertiary qualification in psychology, social work or allied health discipline
- Demonstrated experience in behaviour support services, preferably in disability or mental health sectors
- Strong understanding of positive behaviour support principles and ability to apply them in practice
- Effective clinical assessment and case formulation skills
