**Service Delivery Coordinator Role**

This is a unique opportunity to work with us in delivering high-quality services to our clients. We are seeking a highly skilled and experienced Service Delivery Coordinator to join our team.

The ideal candidate will have a strong background in working with frail older people, people with disability, and their carers. They will be able to demonstrate excellent interpersonal and communication skills, including the ability to initiate and maintain contacts with service providers and community agencies.

We are looking for someone who is passionate about empowering individuals and has a sensitive approach to working with them. The successful candidate will also have knowledge of case management approaches and be able to work as part of a multi-disciplinary team.

This role involves providing planning and case management services to our consumers, operating within funding guidelines, and ensuring that services meet the needs and goals of recipients. The successful candidate will be responsible for monitoring progress, assessing service adequacy, and engaging in program administrative duties.

Key responsibilities include:

  • Establishing new consumers on NDIS/Age Care services
  • Developing service agreements and delivery plans
  • Liaising with service providers and representatives
  • Monitoring client progress and assessing service adequacy
