Job Overview

Seeking a dedicated professional to join our fostering and permanency team, responsible for case management, advocacy, and support services.

Key Responsibilities:
  • Case Management
    • Develop and implement effective case plans, ensuring the best interests of clients are met.
    • Conduct regular reviews to address plan goals and update as necessary.
    • Maintain accurate records and client information.
  • Casework Support
    • Provide emotional support to children and young people in care and their carer families.
    • Facilitate birth family contact and promote engagement with the child's needs.
    • Collaborate with other service providers to ensure comprehensive support.
  • Advocacy and Support
    • Act as an advocate for clients, promoting their rights and interests.
    • Offer guidance and support to carer families to ensure positive placements.
    • Work with clients to identify and address behavioral challenges.
  • Administration and Reporting
    • Prepare reports and documents to a high standard and in a timely manner.
    • Ensure accurate data collection and record-keeping.
Required Skills and Qualifications:
  • Education and Training
    • Minimum qualification of a Diploma in Community Services or a related discipline.
    • Experience in case management and/or working with families or children and young people.
  • Skills and Knowledge
    • Demonstrated understanding of OOHC stakeholder needs and issues.
    • Excellent verbal and written communication skills.
    • Knowledge of Microsoft Office Applications and Needs-Based Framework.
