Job Description

We are seeking a compassionate and dedicated Care Services Coordinator to join our dynamic home care team. This is a rewarding opportunity to play a key role in ensuring that our clients receive the highest standard of care in the comfort of their own homes.

Main Responsibilities:
  • Assessing individual needs and developing personalized care plans
  • Coordinating and managing delivery of in-home care services
  • Regularly reviewing and updating care plans as needed
  • Serving as primary point of contact for clients and their families
  • Collaborating with healthcare professionals and stakeholders for comprehensive care
  • Managing caseload, ensuring timely and effective client support
  • Monitoring quality and safety of care services provided
  • Advocating for clients to ensure appropriate services and support
Key Requirements:
  • Experience in case management, aged care, or related discipline (typically 3 years+)
  • Sound knowledge of Home Care and/or Aged Care industry, including exposure to relevant codes and standards
  • Strong communication skills, demonstrated ability to build rapport with internal and external stakeholders
  • Client-centric approach, willingness to advocate on behalf of clients
  • Proven ability to work independently, make sound judgments, and prioritize tasks in fast-paced environment
  • Experience in budgeting and funds management
  • Demonstrated understanding of risk management protocols, covering hazard identification and incident management
  • Proficient computer and information management skills
