Supporting Our Clients

This role is a key part of our team, dedicated to supporting potential and new clients in understanding their options.

The focus is on drawing on service delivery knowledge and expertise to design a Support Plan that achieves the client's desired goals.

Collaboration with the Client Service Coordination Team will be essential in developing and embedding service design principles and resources across the client journey.

As a supportive team member, you will play a crucial role in person-centred care for our clients.

Key Responsibilities:
  • Supporting potential and new clients to understand their support options
  • Designing a Support Plan to achieve the client's desired goals
  • Working collaboratively with the Client Service Coordination Team to develop and embed service design principles and resources
Essential Skills and Qualifications:
  • Experience in a direct client service delivery role
  • Up-to-date knowledge of the health and/or community services sector
  • Participation in and contribution to improvement and/or change projects
  • Experience in the not-for-profit/community sector, particularly agreements
  • Care Coordination/Case Management desirable
  • Passion for helping and supporting those who are living with a disability and are vulnerable
  • Previous experience in prioritising and managing workload within a busy environment
  • Highly developed interpersonal/communication (written and oral), analytical skills and problem-solving skills
  • Ability to work autonomously and in a team environment
