About Leukaemia Foundation



We are seeking a dedicated professional to join our team at the Leukaemia Foundation. As a Supportive Care Case Manager, you will play a vital role in supporting people impacted by blood cancer.

Key Responsibilities

* Provide best practice supportive care to patients and their families
* Coordinate and integrate supportive care services to improve patient outcomes
* Increase awareness and understanding of blood cancer and supportive care options
* Support patients to manage their physical, emotional, and social well-being

Requirements

* Tertiary qualification in a relevant field (e.g., social work, occupational therapy, nursing)
* AHPRA registration or eligibility for membership of a relevant professional body
* Comprehensive understanding of blood cancer and its impact on consumers
* Knowledge of supportive care services and resources available to blood cancer patients
* Strong interpersonal and communication skills
* Excellent organisational and time management skills
