Aged Care Specialist

The Clinical Aged Care Specialist plays a pivotal role in providing support to older Australians. This position offers flexibility, autonomy, and the opportunity to collaborate with multidisciplinary teams to foster a positive team culture.
Key Responsibilities:- Collaborate with clients, families, and healthcare professionals to identify client needs and develop individualized care plans.
- Conduct comprehensive assessments using the Integrated Assessment Tool (IAT) to determine client eligibility for home support services.
- Review and update care plans to ensure they meet evolving client needs.
- Work autonomously and as part of a broader team to deliver high-quality care within specified timeframes.
- Maintain currency of knowledge on funded and non-funded aged care services.
- Contribute to continuous improvement frameworks to evaluate and enhance assessment processes and outcomes.
- Tertiary qualification in a health-related field (e.g., nursing, occupational therapy, social work).
- Current registration with the Australian Health Practitioner Regulation Agency.
- Australian driver's license and access to a registered, insured vehicle.
- Minimum one year experience delivering aged care services or working with older adults.
