We are seeking a skilled and compassionate Client Outcome Facilitator to join our team at St Vincent de Paul. As a key member of our program, you will be responsible for providing case management, advocacy, and support to clients engaged through our services.
Key Responsibilities- Provide holistic support to clients throughout their journey, focusing on their needs and capacity for independence.
- Develop and implement case management plans that promote client reconnection with their community.
- Operate within the program Service Agreement, including performing shift work as required.
- Collaborate with clients to identify and address their needs, providing information and referral advice as needed.
- Minimum Certificate III in Community Services or related field or Alcohol and Other Drug (AOD).
- Knowledge of and ability to work within relevant legislation and guidelines relating to case management and safety.
- Previous experience as a Support Worker or equivalent in the program area of operation.
- Personal aptitude for working within set guidelines and adhering to policies and procedures.
- Well-developed interpersonal, communication, and organisation skills.
- Ability to deliver advocacy on behalf of clients and maintain confidentiality.
- Understanding of and commitment to the philosophy of St Vincent de Paul Society Queensland.
