Community Care Nurse Opportunity

Deliver high-quality, person-centred care to older adults in their homes and community settings.

  • Provide direct clinical nursing care to our clients in line with best practice nursing procedures
  • Conduct clinical assessments, dressings, medication assessment and catheter changing
  • Develop a holistic nursing care plan in collaboration with the client, carer and relevant healthcare professionals
  • Ensure safe, quality nursing practice to clients with specific needs or complex requirements
  • Liaise with all parties involved in the client's care to maintain holistic health care and wellbeing

Day-to-Day Responsibilities:
  • Providing clinical nursing care to our clients
  • Conducting clinical assessments and developing comprehensive care plans
  • Collaborating with multidisciplinary teams to ensure seamless care delivery
Requirements:
  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar
  • Minimum 3 years post-graduate nursing experience
  • Experience in clinical assessments and holistic care planning
  • Initiative and ability to work independently
  • Strong computer skills, especially Microsoft Office products
  • NSW Driver's License and a reliable vehicle
