Supporting Older Adults in Mental Health

This role offers a rewarding opportunity to provide holistic care to older adults experiencing mental health challenges, chronic conditions, and complex needs.

As a Community Nurse, you will apply primary health care principles to deliver person-centred care within clients' homes and community settings.

You will work collaboratively with a multidisciplinary team to provide early intervention, health education, and recovery-focused support.

Key Responsibilities:
  • Providing direct clinical nursing care to clients in their homes according to best practice nursing procedures
  • Conducting comprehensive assessments, administering medication, and performing catheter changes as required
  • Developing a tailored nursing care plan in collaboration with the client, carer, Care Advisor, and Allied Health professionals where necessary
  • Liaising with all parties involved in the client's care to maintain holistic healthcare and wellbeing
Requirements:
  • A genuine passion for delivering high-quality care to the elderly
  • Current registration with AHPRA following completion of a Bachelor of Nursing or similar qualification
  • Minimum 3 years postgraduate nursing experience
  • Experience in conducting thorough clinical assessments

  • Competitive hourly pay rate and reimbursement for kilometres travelled (99 cents per km)
  • No shift work – working hours are between Monday and Friday, 9am to 5pm
