Job Overview

Purpose of the Role

The primary function of this position is to facilitate a person-centered service that meets the needs and objectives of participants and workers.

This role involves using a holistic approach to identify and review the needs of participants and workers across all phases of recovery and community participation.

You will be responsible for coordinating support to enhance each participant's or worker's capacity to achieve their desired goals, as identified in their individualized plan.

Working collaboratively with internal specialist services, external case managers, health professionals, and other key stakeholders to provide coordinated care and support services.

">Main Responsibilities
  • Utilizing a person-centered approach to identify and review the needs of participants and workers
  • Providing support to enhance each participant's or worker's capacity to achieve their desired goals
  • Collaborating with internal specialist services, external case managers, health professionals, and other key stakeholders
  • Managing service approvals and payments in accordance with agreed-upon needs, assessments, and plans
  • Ensuring adherence to performance indicators that monitor and measure key areas of the role
  • Staying abreast of and understanding the legislative framework and guidelines that impact decision-making
  • Contributing to the design and implementation of internal quality assurance processes, dispute resolution, and continuous improvement initiatives to drive quality and consistency in decision-making
