Welcome to a rewarding career in support coordination. We are seeking an experienced professional to join our team as a NDIS Support Coordinator.

About the Role

The ideal candidate will be responsible for creating partnerships with relevant parties to build on their portfolio of clients in the Albany Region.

Key responsibilities include creating support plans and risk assessments for clients, developing service agreements in line with their budget, and working in a part-time position based in Albany, WA with flexible hours.

Duties and Responsibilities
  • Build Strong Partnerships
  • Acts consistently in accordance with established values and key principles.
  • Works with participants and families: promotes prevention, restoration and connection to create life skills and enable achieving goals.
  • Promotes holistic services that focus on total wellbeing, including physical health.
  • Attends NDIS package review meetings and assists clients to prepare for this.
  • Communicates with the participant's support coordinator and plan manager regularly, to streamline the services.
  • Create and regularly update support plans and risk assessments.
  • Psychosocial recovery coaching.
  • Engages with NDIA staff and LAC's.
  • Ensures the work environment is safe and healthy and complies with all OSH requirements.
Required Skills and Qualifications
  • Minimum Certificate IV in either Mental Health, Disability or qualification deemed equivalent.
  • First Aid and CPR Certificate.
  • Ability to develop ethical work practices.
  • Ability to work within a multidisciplinary team environment.
  • Demonstrated high level written and verbal communication skills.
  • Demonstrated understanding of the person-centered approach.
