Our ideal candidate will be a highly motivated and compassionate professional who is passionate about making a positive impact in the lives of people with disabilities. We are seeking an experienced NDIS Support Coordinator to join our team, someone who shares our commitment to delivering person-centred care while incorporating the principles of compassion, empathy, and kindness.We are looking for someone who possesses excellent communication and interpersonal skills, strong organisational skills and attention to detail, as well as a current driver's licence and access to a reliable vehicle. If you have a minimum 2 years' experience in NDIS Support Coordination or a similar role in the disability sector, we encourage you to apply.The successful applicant will be responsible for working with participants and their families to identify goals and aspirations, coordinating supports and services, monitoring and reviewing participant outcomes, maintaining accurate records, and participating in regular supervision and professional development activities.Key Responsibilities:
  • Participant Management: Collaborate with participants and their families to develop and implement individualised plans that meet their unique needs and goals.
  • Coordination of Supports and Services: Work with external service providers and stakeholders to ensure participants receive necessary support and services.
  • Monitoring and Review: Regularly review participant outcomes and progress towards achieving their goals, making adjustments as needed.
  • Record Keeping and Reporting: Maintain accurate and up-to-date participant records, including documentation of progress, successes, and challenges.
  • Professional Development and Continuous Improvement: Participate in regular supervision and professional development activities to enhance your skills and knowledge.
Requirements:
  • Minimum 2 years' experience in NDIS Support Coordination or a similar role in the disability sector.
  • Knowledge and understanding of the NDIS and disability sector.
  • Excellent communication and interpersonal skills.
  • Strong organisational skills and attention to detail.
  • A current driver's licence and access to a reliable vehicle.
  • Valid NDIS Workers Screening Check and Working with Children Check.
