
Part-time Support Coordinator Role

This is a part-time role that requires strong interpersonal skills and an understanding of support coordination in the context of providing services to people with disabilities.
The successful candidate will be responsible for managing a portfolio of NDIS participants, connecting them with suitable service providers to meet their needs and goals as outlined in their NDIS plans.
Key duties include:
- Managing a case load of NDIS participants and reporting any changes in a timely manner.
- Providing proactive and effective customer communication and building relationships with providers and participant/representatives.
- Identifying and selecting suitable providers for participants and monitoring plan expenditure.
Requirements include:
- Minimum Certificate IV in Community Services/Disability Work or equivalent relevant experience.
- An understanding of support coordination in a human rights context.
- Demonstrated knowledge of sector changes associated with the National Disability Insurance Scheme (NDIS).
- A current driver's license.
- Completion of the mandatory NDIS Worker Orientation Module 'Quality, Safety and You'.
