Job Opportunity

A challenging permanent part-time position is available at a community centre. As a Support Specialist, you will provide tailored assessment and planning to coordinate and assist individuals in accessing various services to improve their wellbeing and achieve their goals.

Main Responsibilities:
  • Facilitate the provision of effective and individualised support to achieve positive outcomes for clients.
  • Assess and document service needs, develop and implement tailored plans, and monitor progress.
  • Liaise with key stakeholders, including healthcare professionals, social workers, and other service providers.
Required Skills and Qualifications:
  • Certificate IV – Social Work or Community Services
  • Experience in Social Housing, Community Services or related field
  • High level negotiation skills and ability to liaise with key stakeholders
  • Competence in service assessment and documentation (including service planning)
