
Family Support Coordinator

We are seeking a Family Support Coordinator to join our team in the Sydney Children's Hospitals Network.
Job DescriptionThe successful applicant will work with families to identify unmet social needs and support them to access appropriate services.
- The Family Support Coordinator will provide practical support, including navigating services, making referrals, completing forms, and booking appointments.
- Provide supportive care to families to help them access necessary services
- Navigate complex service systems to secure timely assistance
- Work collaboratively with health professionals to ensure seamless delivery of care
- Certificate IV in Allied Health Assistance or working towards similar certification
- Ability to work independently and collaboratively with health professionals in a multidisciplinary team environment
- Prioritise workload, meet deadlines, and complete activities as delegated by supervising Allied Health Professional
- Experience working with children and their families within a healthcare setting or alternative capacity

Please note that this is a temporary part-time position, requiring up to 20 hours per week over three days between Monday and Thursday.
