We are seeking experienced professionals to join our team as Social Workers. In this role, you will work with a dedicated Allied Health team, providing social work services in acute and sub-acute settings across various sites.

Key responsibilities include:

  • Providing social work services in acute and sub-acute settings
  • Maintaining high levels of communication with patients, visitors, and colleagues
  • Delivering exceptional patient care through psychosocial assessments, risk assessments, complex discharge planning, and family support
  • Working autonomously and collaboratively as part of a friendly and energetic team

You must possess excellent communication and interpersonal skills, dedication to safe best practice and quality care, and an unwavering commitment to delivering patient-centred care.

Required skills and qualifications include:

  • AASW accredited Bachelor degree or entry-level Master's Degree of Social Work
  • Minimum of four years' experience in a healthcare setting
  • Good organisational skills and a positive, enthusiastic attitude towards work
  • Experience in working with culturally and linguistically diverse communities
  • Able to work autonomously
  • Friendly, outgoing, and reliable demeanour
  • Well-developed clinical assessment and complex discharge planning skills
  • Current Victorian drivers licence
