
Psychosocial Care Specialist

We are seeking experienced professionals to join our team as Social Workers. In this role, you will work with a dedicated Allied Health team, providing social work services in acute and sub-acute settings across various sites.
Key responsibilities include:
- Providing social work services in acute and sub-acute settings
- Maintaining high levels of communication with patients, visitors, and colleagues
- Delivering exceptional patient care through psychosocial assessments, risk assessments, complex discharge planning, and family support
- Working autonomously and collaboratively as part of a friendly and energetic team
You must possess excellent communication and interpersonal skills, dedication to safe best practice and quality care, and an unwavering commitment to delivering patient-centred care.
Required skills and qualifications include:
- AASW accredited Bachelor degree or entry-level Master's Degree of Social Work
- Minimum of four years' experience in a healthcare setting
- Good organisational skills and a positive, enthusiastic attitude towards work
- Experience in working with culturally and linguistically diverse communities
- Able to work autonomously
- Friendly, outgoing, and reliable demeanour
- Well-developed clinical assessment and complex discharge planning skills
- Current Victorian drivers licence
