Job Description:

This position involves delivering evidence-based physiotherapy assessments and interventions to clients under aged care funding programs, with a focus on reablement and restorative care. Your work will help older people maintain or regain independence, mobility, and confidence, enabling them to stay safe and connected within their communities.

Key Responsibilities:

  • Conduct thorough physiotherapy assessments to identify individualised needs and develop targeted intervention plans
  • Deliver high-quality physiotherapy services that are tailored to meet the unique requirements of each client
  • Collaborate effectively with multidisciplinary teams to ensure seamless care coordination and optimal outcomes for clients
  • Develop and maintain strong relationships with clients, carers, and other stakeholders to promote trust and open communication

Requirements:

To be successful in this role, you will require:

  • A degree in Physiotherapy with current AHPRA registration
  • Proven ability to assess physiotherapy needs and develop individualised programs for aged care clients in the community
  • Postgraduate experience working with diverse aged care populations, including rehabilitation and community settings
  • Comprehensive understanding of reablement and restorative care models, with a focus on promoting independence and quality of life
  • Excellent written and verbal communication skills, with the ability to adapt to varying communication styles and contexts
  • Demonstrated computer literacy, with proficiency in relevant software applications and systems

  • Current first aid certificate or willingness to obtain
  • Unrestricted driver's licence and access to a reliable, registered vehicle with minimum Third-Party Property Damage car insurance
