Role Summary

About the Position

We are seeking a compassionate and resilient professional to provide person-centred support to individuals transitioning from hospital to community living.

This role involves working on a 24/7 rotating roster, including nights, weekends, and sleepovers, to help people achieve independent living.

Key Responsibilities
  • Support clients in returning to the community and maintaining independent living
  • Engage clients using evidence-based practices, including motivational interviewing and trauma-informed care
  • Plan and deliver group programs to facilitate recovery and skill-building
  • Provide education, information, and referrals to families and carers
  • Collaborate with community services to ensure coordinated and integrated support
  • Contribute to daily residential operations, including housekeeping duties
Requirements
  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field
  • Current First Aid and CPR certificates
  • Experience working with people with mental health challenges and complex needs, including families and carers
  • Proven capability to implement therapeutic frameworks and practices, including risk mitigation and family inclusive practice
