Compassionate Case Manager

We are seeking a dedicated professional to work with clients and their families/carers to develop, refine, and achieve their goals.

This is a full-time role where you will collaborate with local Support Coordinators, Allied Health professionals, and clinical teams to provide information and support to clients regarding the use of their funds.

Key Responsibilities:
  • Work collaboratively with clients to establish, revise, and implement care plans that meet their needs and goals.
  • Provide timely and accurate case noting, service agreements, and client reviews that reflect forward planning in line with participant's funding plans.
  • Conduct annual reviews for all clients - revise care plans, goals, service provisions to quality standards, and budget expenditure and maximization.
  • Ensure service provisions are conducted ethically, fairly, and within statutory legal and contractual requirements.
  • Compliance with health policies and procedures to ensure a safe working environment and detailed, timely incident reporting.
Requirements:
  • Understanding of NDIS and Home Care Packages.
  • Previous case management experience.
  • Demonstrated knowledge of community care funding models.
  • Exceptional customer service, negotiation, and influencing skills.
  • Excellent ability to connect with multiple stakeholders and think commercially.
  • Strong interpersonal skills and high levels of resilience.
  • Driver's license and access to own vehicle (company car available for use in business hours).
  • Professional verbal and written communication skills.
  • Current Driver License, First Aid/CPR, Blue Card, and Police Check.
  • Reside in the Cairns area.
