We are seeking a dedicated and compassionate professional to join our team as a Case Manager.

Job Overview

This role involves working closely with Support Coordinators, Allied Health professionals, and clinical teams to develop and refine client goals.

The ideal candidate will possess exceptional customer service skills, strong interpersonal skills, and high levels of resilience.

A key aspect of this position is ensuring accurate case noting and service agreements, as well as conducting annual reviews and revising care plans.

Responsibilities
  • Collaborate with Support Coordinators, Allied Health professionals, and clinical teams to create personalized client care plans.
  • Provide expert guidance and support to clients and their families/carers.
  • Ensure seamless coordination of community care services.
Requirements

To be successful in this role, you will need:

  • Previous experience in case management or a related field.
  • Strong understanding of community care funding models.
  • Excellent communication and interpersonal skills.
