About the Position

The Complex Care Team provides services and supports to both Out of Home Care (OOHC) / Voluntary OOHC and National Disability Insurance Scheme (NDIS) participants allowing a dual role across Youth and Disability Support. The Complex Disability Support Worker is responsible for developing, assisting, and supporting clients in various locations such as houses, schools, hospitals, community centers, and emergency accommodation on a daily basis.

Key Responsibilities
  • Develop and implement individualized plans to meet the unique needs of clients.
  • Provide safety and support to clients while identifying and addressing growth needs.
  • Collaborate with a variety of stakeholders, including family members, healthcare professionals, and other support workers.
About You

We are seeking a dedicated and compassionate individual who shares our commitment to delivering exceptional care and support. To be successful in this role, you will have:

  • A qualification in Community Services (Cert IV) or relevant experience.
  • A strong interest in youth work, mental health, and the disability industry.
  • An understanding of strength-based trauma-informed practice frameworks.
  • The ability to obtain the NDIS Workers Screen Check prior to commencement.
  • A willingness to work shift work, regular sleepovers/active nights, weekends, and public/school holidays.
