About the Role

We are seeking a highly motivated and passionate Lead Therapist to join our team as a social model practitioner. As a key member of our interdisciplinary team, you will work closely with participants and their support teams to develop new skills and habits, problem-solve challenges, and champion the role of social model practitioners in breaking down barriers for people with disabilities.

In this role, you will use evidence-based practice and professional reasoning to recommend the most appropriate team of supports, therapy, duration, location, and timing. You will also enhance people's capacity to make their own life choices and increase their independence, while ensuring safeguards are in place to protect the rights and well-being of the most vulnerable.

Responsibilities
  • Conduct initial assessments using our LEAD evidence-based assessment tool
  • Liaise with the interdisciplinary team to wrap the right supports around the participant and their family
  • Support individuals in their everyday environment – home, work, school, or community
  • Capacity build their support team to provide a capable environment
  • Attend peer group supervision/mentoring and professional learning to grow expertise and skills
  • Participate in weekly team meetings and share stories of impact
  • Complete reflective practice to develop evidence-based plans
Requirements
  • Hold a Bachelor or Masters of Disability (or equivalent) and be eligible to be a member of DEAI
  • Be required to submit a suitability assessment and be deemed a Behaviour Support Practitioner by the NDIS Quality and Safeguards Commission
  • Show empathy and understanding, while being open and non-judgmental
  • Be driven to see high-impact outcomes in people's lives
  • Obtain an NDIS Worker Screening Check & Working with Children's Check
  • Hold a valid driver's license

Salary

$83,582 - $100,342 per annum, plus annual leave loading and superannuation.
