Emergency Department Care Coordinator

This exciting opportunity involves working with our multidisciplinary team to deliver patient-centered care in the Emergency Department.
As a skilled and experienced Care Coordinator, you will be responsible for:
- Providing timely and comprehensive assessment and care coordination for patients with complex needs;
- Collaborating with patients, their carers, and members of the healthcare team to achieve assessment and care planning;
- Supporting discharge planning and transition into the community;
To succeed in this role, you will need:
- A nursing or allied health qualification relevant to the cohort;
- Minimum 2 years' experience in a relevant acute setting;
- AHPRA registration where applicable, and/or eligibility for membership of a professional association;
- Current Victorian Driver's License;
