Emergency Department Care Manager

We are currently seeking a skilled and dedicated Care Coordinator to join our Emergency Department team.
As a key member of the ACE ED Team, you will be responsible for providing expert care coordination, rapid allied health services, and integrated care planning to optimize patient outcomes and reduce presentations/admissions.
To succeed in this role, you will require:
- A tertiary qualification in a relevant field (Occupational Therapy, Social Work, Physiotherapy, Nursing) and eligibility for discipline-specific registration;
- Minimum 2 years' experience in a healthcare setting (acute, subacute, community-based care);
- Strong skills in assessment, discharge planning, case conferencing, and care planning;
- Excellent communication and teamwork abilities;
- A commitment to lifelong learning and growth within the Emergency Department environment;
- A passion for delivering exceptional patient care.
