Clinical Case Manager

The Drug Court of Victoria seeks an enthusiastic and highly organised Case Manager to join the Melbourne team.
This pivotal role assists individuals in improving their lives by providing clinical advice, treatment and referral services for the Drug Court and its participants, in line with model principles and practices.
Key Responsibilities- Assess potential participants referred to the Drug Court and develop treatment plans to address substance use and associated issues.
- Develop individual treatment plans for participants on a Drug Treatment Order to address their substance use, psychosocial history and suitability for inclusion in and ongoing participation in the program.
- Participate in case conferences and provide expert advice to the Court on offender psychosocial history, substance use and suitability for inclusion in and ongoing participation in the program.
- Provide assessment and progress reports for the Drug Court Magistrate.
- Develop, manage and maintain relationships with internal and external stakeholders, including Court Services Victoria, court staff, judiciary, community organisations, drug treatment agencies, peer agencies and other Victorian Government Departments and Non-Government Organisations.
- Contribute to the development and progress of the Drug Court program, including the development and continuous improvement of clinical policies, procedures and assessment tools.
- Tertiary qualification in health, social work, welfare or related field with emphasis in AOD is required.
- DHHS Accreditation is desired.
