Support Services Coordinator

We are seeking a Support Services Coordinator to join our team of dedicated professionals. This role provides essential coverage for Care Partners during periods of planned or unplanned absence, ensuring continuity and consistency in the delivery of person-centred care to participants in our Community Care program.

Job Description:

The primary focus of this role is to support participants and their registered supporters and family members, identifying needs, goals, personal preferences, and existing supports, and providing timely information on available services.

This involves communicating with relevant stakeholders involved in the delivery of services regarding the participant's needs and wellbeing to action the care plan and associated recommendations in a timely manner.

In addition, you will work within a team-based model of care, ensuring clinical decisions are escalated appropriately and timely, and participate in shared planning and problem solving to improve the participants' health outcomes.

You will also support participants to make informed decisions, including respecting their right to take risks, engaging with registered supporters and carers as appropriate, and as per the supported decision-making framework.

Required Skills and Qualifications:

  • A tertiary qualification in Health or a related field, such as Individual Support – Ageing, Health Services Assistance, Aged Care, Disability, or Community Services.
  • A Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience.
  • Formal training in trauma-informed care, cultural safety, or restorative practice.
  • A strong background in aged care - home care, care management, or a healthcare setting.
  • A sound understanding of compliance in Aged Care and NDIS, including quality standards and Work, Health and Safety (WHS) regulations.
  • Skilled in using care management or Customer Relationship Management (CRM) systems.
