The Role: Health Linker
About the PositionWe are seeking a skilled and compassionate individual to fill the role of Health Linker. In this position, you will have the opportunity to make a meaningful impact in the lives of others by providing holistic, coordinated support.
As a Health Linker, you will work closely with clients, allied health providers, general practitioners, and community services to deliver person-centred support plans that enhance health and well-being.
Key Responsibilities- Develop and implement care plans tailored to individual client needs
- Establish connections between clients and vital health and social services
- Craft personalised support plans that promote overall health and well-being
- Tertiary qualifications in Nursing, Aboriginal Health, Allied Health, Community/Human Services, Social Work/Social Science or equivalent
- Understanding of broader determinants of health including social, economic, and environmental factors
- Experience in co-developing person-centred support plans and advocacy skills
