About the Role

This innovative role combines clinical expertise with virtual technologies to deliver high-quality, patient-centred care directly into the community. As a Community Nurse, you will assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing for individuals transitioning from hospital to home.

  • You will work collaboratively with patients, families, and a multidisciplinary team to provide comprehensive care that addresses health promotion, early intervention, and continuity of care.
  • Your strong communication skills will enable you to build trust with patients and their families, ensuring safe, timely, and effective discharge planning.

Benefits include opportunities for career growth, professional development, and work-life balance through accrued days off, annual leave loading, and paid parental leave. Additionally, you will have access to salary packaging, relocation assistance, and a range of employee benefits including discounted gym memberships and private health insurance.

To be successful in this role, you will possess a current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and hold a current Australian Drivers Licence.
