About the Role

The Primary Health Care Specialist in our Child Health Outreach Program plays a vital role in working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs.

This position has a strong clinical focus on outreach work, including liaison with internal and external stakeholders involved in the child and family's care. The successful candidate will have the opportunity to make a real difference in the lives of children and their families in Central Australia.

Key Responsibilities
  • Coordinate care to optimise health outcomes for children with complex chronic healthcare needs
  • Work collaboratively with children, families, and healthcare providers to develop and implement individualised care plans
  • Provide education and support to families to promote healthy lifestyle choices and self-management of chronic conditions
  • Liaise with internal and external stakeholders to ensure seamless coordination of care
Requirements

To be successful in this role, you will need:

  • Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health
  • Working knowledge of Child Health and an understanding of current practices, theories, and values
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People
