About the Role

The successful candidate will work collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.

This role has a strong clinical focus on outreach work including liaison with internal and external stakeholders involved in the child and family's care.

Key Responsibilities
  • Provide comprehensive nursing care to children and their families in a community setting;
  • Develop and implement individualised care plans to meet the unique needs of each child;
  • Liaise with multidisciplinary teams, including general practitioners, specialists, and other healthcare professionals, to ensure seamless care coordination;
  • Collaborate with community services and agencies to address social determinants of health and improve health outcomes for children and their families;
Requirements
  • Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA);
  • Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health;
  • Working knowledge of Child Health and an understanding of current practices, theories, and values;
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People;

To be successful in this role, you will have a strong passion for working with children and families, excellent communication and interpersonal skills, and the ability to work effectively in a team environment.

You will also possess a high level of emotional intelligence, cultural competence, and understanding of the social determinants of health that affect Aboriginal and Torres Strait Islander peoples.

What We Offer

In return for your expertise and dedication, we offer a competitive salary package, including:

  • A base salary of $103,759 - $110,838 per annum;
  • A district allowance;
  • Superannuation;
  • Leave loading;
  • An estimated total effective package of $126,455 - $134,420 per annum;*

*Total effective package includes: base salary, district allowance, superannuation, leave loading, and estimated tax saving from salary packaging options.
