Job Opportunity

We are seeking a highly skilled and experienced occupational therapist to join our multidisciplinary team in North Queensland. This is an exciting opportunity to work with older Australians and support them in regaining their physical and psychosocial function following a hospital stay.

The role will involve providing occupational therapy assessment and intervention for clients participating in the Transition Care Program, as well as working with a 10% NDIS caseload. The goal of this role is to restore function and independence, and to support clients in achieving their goals.

About the Transition Care Program
  • The Transition Care Program (TCP) is a joint Commonwealth and State-funded initiative designed to support older Australians in regaining their physical and psychosocial function following a hospital stay.
  • Delivered nationally through State and Territory Health Departments, the program offers short-term restorative care aimed at enhancing independence and avoiding premature entry into residential aged care.
  • Services may include nursing support, personal care, domestic assistance, transport, and low intensity allied health therapies such as Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, and Social Work.
Responsibilities
  • Provide occupational therapy assessment and intervention for TCP clients and 10% NDIS caseload, with the goal of restoring function and independence.
  • Collaborate with the TCP multidisciplinary team to deliver goal-oriented, time-limited therapy.
  • Document progress, set goals, and complete discharge planning in a timely manner.
Requirements
  • Experience: 2 years of experience or more in occupational therapy.
  • Driving license; home-based service.
