Overview

About the Transition Care Program (TCP)
  • The TCP is the lead allied health service provider for the program within the region; established in 2005, it is a joint Commonwealth and State-funded initiative designed to support older Australians in regaining physical and psychosocial function following a hospital stay.
  • Delivered nationally through State and Territory Health Departments (Approved Providers under the Aged Care Act 1997), the program offers short-term restorative care (up to 12 weeks) aimed at enhancing independence and avoiding premature entry into residential aged care.
  • The program is goal-oriented, time-limited, and therapy-focused, with services tailored to each individual. These may include nursing support, personal care, domestic assistance, transport, and low intensity allied health therapies such as Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, and Social Work.
  • In the region, TCP offers 46 community-based places, servicing Townsville, Magnetic Island, Ingham, Charters Towers, Ayr, Home Hill, and Mt Isa.
  • Experience required/Level of the role: 2 years of experience and above; there are 4 other OT team members based in Townsville.

Responsibilities
  • Provide occupational therapy assessment and intervention for TCP clients and 10% NDIS caseload, with the goal of restoring function and independence.
  • Collaborate with the TCP multidisciplinary team and Townsville-based staff to deliver goal-oriented, time-limited therapy (up to 12 weeks).
  • Document progress, set goals, and complete discharge planning in a timely manner.
Qualifications
  • Experience: 2 years of experience or more in occupational therapy.
  • Other requirements: Driving license; TCP home-based service.
